In order to prepare an employee benefit proposal for your consideration, please send us the following
census information:

A copy of your most recent health insurance premium statement & policy benefit schedule (deductible, co-pays, drugs, dental, STD, LTD, life
insurance, ~125/129 cafeteria plan, 401k, etc.).
We need to compare 'Apples and
Apples' to be accurate.
An employee census of all eligible participants, not just those participating. Sex, date of birth, married/single, number of children, job class and salary.
Any information about the past few years claims,
your last renewal sheet, and loss run information, or pending claims (i.e. any currently pregnant employees or pending heart transplants, etc.)
A description of what you "like" and "dislike" about your current program, and a "wish list" if price were no object.
